Patient Information

Your First Appointment

This session is a meet and greet, to build some rapport with you, understand your goals and desired outcomes thoroughly and provide you with some initial assistance. We ask that you please bring any further evidence/ information to this session that you haven’t already passed to our friendly admin team.

We request that all patients arrive about 15 minutes prior to their Initial Appointment time to ensure that all initial appointment forms are completed in time for the appointment.
If relevant, please bring:

  • Your doctor’s referral
  • Relevant imaging reports (eg x-ray, Ultrasound Scans or MRI)
  • Discharge summary from the hospital
  • Current exercise regimes from previous treatments
  • Completed patient registration form. You can download a copy of the form here.

Your Second Appointment

This session is where our therapists will sit down with you after taking into consideration all of the information and your desired outcomes to deliver you their therapy plan and the best approach to reaching these goals in a realistic and achievable time frame. Furthermore, some coping mechanisms and strategies will begin to be implemented within this session.

Your Third & Subsequent Appointments

This is where you find the process really kicking into full swing, any reports required are underway, the therapy plan is in full delivery and you will begin to see changes and goals achieved moving forward from this session. Any communications/links between Support Coordinators and yourself have been established and clear communication for outcomes are flowing.

*Note: Each session and participant is different and this is just a general outline of how sessions are generally conducted and what to expect.

Private Health Insurance

If you have a private health fund, you can use this to assist in the payment of your appointments. Each health fund provides different amounts of rebates. If you are unsure of your entitlement, please consult with your health fund.

Payment is requested on the completion of each consultation. We have a HICAPS terminal to process your claim and EFTPOS for your convenience.

A doctor’s referral is not required to claim our services.

NDIS

We offer services under the NDIS to patients whose funds are self-managed or managed by a Plan Manager. We will work closely with you to help you achieve your NDIS plan goals.

Medicare

We welcome referrals under Medicare for patients with chronic medical conditions and complex care needs. Please talk to your doctor to find out if you qualify for this type of referral.

Please note, a co-payment fee is payable by the patient, which will be conveyed to you when you book an appointment using this referral type. Payment is requested upon completion of your appointment. Eftpos is available for your convenience.

Department of Veteran Affairs (DVA)

Gold Card Veterans and War Widows are very welcome. There must be a current referral (D904) from your GP or specialist in order for us to claim treatment through the Department of Veteran’s Affairs.

White Card holders are also very welcome, but to claim physiotherapy treatment under the Department of Veteran’s Affairs, the treatment must be for the eligible condition, which must be stated on the doctor’s referral (D904).

If the veteran is over 80 years old, and transport to the practice is difficult, then, upon request, a taxi can be arranged through DVA.

We also assist in supplying goods, such as walking aids, to eligible veterans.

Work Cover

If you have a current, accepted Workcover QLD claim or a GP or Specialist referral that allows you access to allied health services, you may be eligible for services that Workcover covers. Please provide us with a copy of your Workcover Certificate or GP referral so we can check your eligibility.

FAQs

When can I get in to see your OT/ Psychologist?

Currently, some of our Practitioners have no wait times and can allocate you an appointment in the near future. Though, keep in mind the complexity of your situation can also depend on where we allocate your appointment time. We wish to provide you with the best support outcomes.

How much will it cost?

Depending on the service, you can always refer directly back to the NDIS guide to understanding how pricing works. Please ask our administration team as each service and volume of services are different. Each will be highlighted in the Service Agreement and Schedule of Support.

Are your therapists Male or Female?

Depending on the profession, though we have both Male and Female team members to facilitate your needs and will provide you with the best support and of course your choice.

What happens if I cannot attend or am running late?
Conquer Health Alliance has set aside an appointment time for you. We run strictly on time, most of the time so if you are too late for us to accommodate your treatment needs, you will have to reschedule your appointment. Should you be late for your appointment, or fail to attend a scheduled appointment, then our non-attendance policy may be enforced.
